Summer Physical Form Requirements

Please read this (even if you have been around for a while) to make sure your filing the correct form.

Each unit member (youth and adult) is required to have a physical on file with the troop. Use the chart below to figure which form is needed, based on the activities(s) you are participating in. (Please use the form farthest to the right.)

The class II form is valid for 3 years with re-certification; please check, you may not need to visit a physician. The class III form is valid for 1 year only and must be re-accomplished each year.

However both forms have space for certifying updates.

* Youth and Adult members participating in this summers Philmont experience are required to file a specific form. This form may also be used for Summer Camp. Please see Mr. Hoy if you have not received the form.

A Note about Summer Camp: Please note when reading information published by the Great Trail Council that there are references to the "Wednesday Family Night." Troop 380 DOES NOT PARTICIPATE IN THIS EVENT, please do not come out to camp on Wednesday night, as you will be asked to leave. It is the practice of the Troop that once your son arrives at Summer Camp he is there for the entire week, and DOES NOT DEPART. In this way we may capitalize on the time available for advancement and work within the Troop. Also by not participating in Wednesday nights you help to prevent the "reoccurrence" of the dreaded home-sickness that seems to occur especially with the 1st and 2nd year campers. Please help us in STAYING AWAY FROM CAMP IF YOUR NOT PART OF THE DESIGNATED LEADERSHIP OF THE UNIT DURING THE TIME THE TROOP IS AT CAMP. Questions related to this policy should be directed to Mr. Hoy or Mr. Chandler.
